Please complete this form to request counselling at Family Services Durham (FSD).
FSD provides individual, relationship/couple and family counselling and psychotherapy to people who live, work or own property in Durham Region. We can help with a range of issues including mild to moderate mental health and substance use challenges (e.g. anxiety and depression), family or relationship conflict, and stress. Service is offered in-person, by telephone or by video. Fees are determined on a sliding scale based on your income.
Counselling may be provided to children 6 years of age and older. Children between 6 and 11 years of age are provided with service within family counselling with their parent(s)/caregiver(s).
