Welcome to the SmartStart Hub Online Referral Form for KidsAbility.

KidsAbility is the SmartStart Hub for Waterloo Region, City of Guelph, and Wellington County. We provide a range of therapy services to children and youth aged birth to 18 (or until age 21 if attending a publicly funded school in our area).

What is a SmartStart Hub?
A SmartStart Hub supports and connects families with services for concerns related to their child’s development and day-to-day functioning related to development. A SmartStart Hub can support families to determine next steps when a caregiver or supporting professional has concerns about a child or youth’s ability to speak with and be understood by others, to move around, to perform daily tasks such as feeding, eating, toileting, and dressing, or other development concerns.

Depending on a families' needs, SmartStart Hub services may be provided by KidsAbility or by other community Partner Agencies

Who should connect with a SmartStart Hub by completing this form?
The SmartStart Hub is for families with concerns about their child’s development who:

  • Are not sure what their child’s development needs are or not sure where to start.
  • Are not sure what services to seek or how to access them.
  • Feel they would benefit from a discussion about their child’s development/strengths/needs.
  • Feel they would benefit from referral(s) to appropriate services, and access to family education and family supports.
