Outreach Support provides scheduled visits from a Personal Support Worker (PSW) in your home.

Housing with PSW Support is provided in properties owned or operated by Thrive Housing and Support, offering PSW assistance from a few hours a week to 24-hour care. Thrive has housing in Peterborough and limited locations outside. Applicants needing both housing and support must also apply to Housing Access Peterborough or City of Kawartha Lakes Housing for Lindsay locations.

This application is not for individuals who need housing only.

High-risk senior (as determined by raiCHA assessment)

Acquired brain injury (as diagnosed by a medical professional)

HIV/AIDS (as diagnosed by a medical professional)

The applicant requires personal care (help with personal hygiene, eating, dressing or mobility).
Applicants must require personal care to be eligible for Thrive PSW Services. In addition to personal care, we can also offer homemaking services such as help with housecleaning, laundry, shopping and preparing meals. *

The applicant lives in Peterborough City or County or the City of Kawartha Lakes. *
