Service Information & Eligibility

Rehabilitative Care (Low Intensity Inpatient Rehab)

Short-term inpatient rehabilitation for adults who need reconditioning or retraining following illness, injury, surgery, or a decline in function. Care focuses on restoring mobility, strength, endurance, and independence with a typical average stay of 4 weeks. Patients must be medically stable and able to participate in daily therapeutic activities.

Long-Term Complex Medical Care

Inpatient care for individuals with chronic or progressive health conditions that cannot be managed at home, in long-term care, or in an acute hospital setting. This service supports patients with high medical needs through an interdisciplinary approach focused on comfort, quality of life, and ongoing clinical management. Patients admitted typically remain in care long-term, and must have been deemed ineligible for traditional long-term care due to the complexity of their condition.
